What Does a DOT Physical Include?

  • Review of medical history: Assessment of past and current conditions, including heart disease, diabetes, neurological disorders, sleep apnea, and any medications that could affect driving.
  • Vision test: Measures visual acuity (at least 20/40 in each eye, with or without correction), peripheral vision, and the ability to distinguish red, green, and amber colors (colors of traffic signals).
  • Hearing test: Evaluates the ability to hear a forced whisper from at least five feet away.
  • Blood pressure and pulse check: Screens for high blood pressure and irregular heart rhythms.
  • Urinalysis: Tests for protein, sugar, and other markers that could indicate diabetes, kidney disease, or other underlying conditions.
  • Physical examination: A full-body evaluation that includes:
  • Reflexes, balance, coordination, and muscle strength
  • Joint mobility, back and spine health, and signs of pain or limitations that could impact the ability to drive
  • Heart and lung function
  • Screening for hernias, organ enlargement, or tenderness

  • Will insurance pay for DOT physicals?

    Employer-sponsored health insurance may cover DOT physicals, but it typically does not. This is because they are considered a regulatory requirement rather than a medically necessary procedure. However, at Impact Family Wellness we do not require insurance. We offer clear, upfront pricing with no hidden or surprise fees.

  • How much is a DOT physical?

    For members: DOT physicals are $95 (in addition to their monthly membership fee).

  • How long does a DOT physical usually take?

    Most DOT physicals take 30 to 45 minutes, depending on medical history and any additional evaluations needed. If you pass, you will receive your DOT medical card the same day.

  • What will make you fail a DOT physical?

    You may fail a DOT physical if you have:


    • Uncontrolled high blood pressure (above 180/110).
    • Severe vision or hearing impairment that cannot be corrected with glasses, contacts, or hearing aids.
    • Uncontrolled diabetes with dangerously high blood sugar levels.
    • Recent heart conditions (heart attack, stroke, or untreated heart disease) without proper medical clearance.
    • Drug or alcohol use issues or a history of failed drug tests.
    • Neurological disorders or musculoskeletal conditions that impair your ability to drive safely.

  • What is the blood pressure limit for a DOT physical?

    • 140/90 or lower → Passes without restrictions (valid for up to 2 years).
    • 140/90 - 179/109 → May qualify for a shorter certification period (3-12 months).
    • 180/110 or higher → Automatic disqualification until blood pressure is under control.
